2012 Strategic Goals

Sustainability:

1. Demonstrate CHC value through collection of meaningful data.

2. Assure that Health Center reimbursement supports their capacity to provide essential, high quality services to vulnerable populations.

3. Provide robust operational and leadership Technical Assistance to assure sustained capacity and quality of care in CHC's.

4. Enhance internal communication with OPCA members.

5. Assure OPCA continues to build capacity and resources necessary to meet the needs of our members.

Innovation:

1. Spread and sustain the patient centered transformation of health care in CHC's to a higher quality, cost effective, equitable and accessible PCPCH.

2. Assure health reform movement towards alternative payment recognizes barriers to health confronted by vulnerable patients we serve that exacerbate their medical severity, including but not limited to mental health, income, housing, educational, racial, ethnic and cultural factors, geographic isolation and lack of transportation.

3. Assure CHCs are prepared for evolving developments in health reform transformation, the CCO environment in particular, and that they are supported with knowledge and best practices to enhance their success in local partnerships.

4. Advance the concept of the "Quadruple AIM": Cost, Quality, Access and EQUITY.

Growth:

1. Expand and deepen the advocacy capacity of health centers to influence health reform.

2. Enhance the workforce capacity of health centers to meet the growing and evolving needs of their communities, with an eye to emerging needs (e.g., IT/data support).

3. Assist current members and new communities in expansion efforts to meet needs in an environment of constrained resources.
