Oregon's community health centers are leading a progressive change in health care delivery by pioneering the patient-centered primary care home model of care.

 Key elements of the model include:
  • Strong personal relationships between patients and health care professionals.
  • Culturally competent care from teams of professionals, including case managers and behaviorists.
  • "Off hours" access to health care providers via telephone, text, and email.
  • Use of evidence-based medicine.
    In a number of states, this model has been shown to result in better health at lower cost.

    The Safety-Net Medical Home Initiative
    In 2009 - working with partners CareOregon and the Oregon Rural Practice-Based Research Network - OPCA secured one of five national demonstration project grants, to support the development of the patient-centered primary care home model in Oregon. The Safety-Net Medical Home Initiative grant is funded by The Commonwealth Fund and supported by Qualis Health. Ten community health centers and five additional safety-net clinics in Oregon are participating in the four-year, $500,000 initiative.

    The patient-centered primary care home model integrates mental health into primary care services. A team-based approach to care is central to the primary care home model.
