Oregon's community health centers are leading a progressive change in health care delivery by pioneering the patient-centered primary care home model of care.

The Safety-Net Medical Home Initiative
In 2009 - working with partners CareOregon and the Oregon Rural Practice-Based Research Network - OPCA secured one of five national demonstration project grants, to support the development of the patient-centered primary care home model in Oregon. The Safety-Net Medical Home Initiative grant is funded by The Commonwealth Fund and supported by Qualis Health. Ten community health centers and five additional safety-net clinics in Oregon are participating in the four-year, $500,000 initiative.
