OPCA provides technical assistance (TA) and training for all Federally Qualified Health Centers (FQHCs) and Look-A-Likes in the state of Oregon.  Check our Calendar of Events for details.

OPCA's primary care learning community program builds on clinics' existing work with health disparities collaboratives.  That work focuses on building registries and improving care for patients with chronic conditions.  The learning community broadens this approach by promoting the Patient-Centered Medical Home Model.  Our Safety Net Medical Home Initiative expands the model in Oregon.  Through the initiative, 10 of our members are transforming the delivery of primary care through a focus on innovation, teamwork and quality.

Our Patient Self-Management Collaborative represents a unique opportunity for community health centers to improve outcomes for patients with chronic health conditions and move toward key elements of the medical home model.  The collaborative aims to increase referrals of appropriate patients with chronic diseases to community-based self-management resources like Living Well with Chronic Conditions, Tomando Control de su Salud, the Oregon Tobacco Quit Line and the Arthritis Foundation exercise programs.

 


 

 

OPCA provides technical assistance (TA) and training for all Federally Qualified Health Centers (FQHCs) and Look-A-likes in Oregon - to help improve the health status and quality of health care provided to their patients. Our quality improvement programs support our mission to increase health equity and access to comprehensive health care for low-income and vulnerable people in Oregon. OPCA combines strategies to ensure that FQHCs remain viable while assisting them to expand their services and community outreach. We organize our TA and training activities under three overarching goals:

  1. Strengthen existing health centers,
  2. Manage the growth of new and existing health centers, and
  3. Promote and provide TA and training support for quality improvement and excellence.

The number of requests for OPCA's TA and Training Services has grown exponentially in past years due to the weak economy, continuing health center growth, and increasing momentum for Primary Care reform. OPCA has responded by greatly expanding its Technical Assistance Program beyond viability and growth assistance to include a focus on the Primary Care Home model that provide quality improvement methods to operationalize a systematic approach to health care delivery and health outcomes for FQHCs in Oregon.

OPCA continues to be committed to help FQHCs strengthen, to grow and to improve their quality of care by addressing multiple areas of health center excellence through trainings, quality improvement programs, on-site technical assistance, and advocacy.
