We are currently seeking a Panel Manager to join our West Salem Medical Office in Salem, Oregon!
YOUR ROLE AT NWHS
Our Panel Manager collaborates with a multidisciplinary team to improve health outcomes of patients through population health. As our Panel Manager, you will assist our Ambulatory Care leadership in identifying population management priorities interventions and campaigns. You will participate in continuous process improvement activities to improve the quality of care and chronic disease management. This position facilitates our patients' access to appropriate care services and follow-up care related to population health management. You will also actively participate in quality improvement meetings and events, providing support to quality improvement in patient and provider feedback. Additionally, your role performs out-reach and in-reach population management.
Requires a high school diploma, GED, or equivalent
Must have computer familiarity and typing skills
Must be able to interact effectively and courteously with patients and staff
Must be able to work effectively in a fast-paced environment
Bilingual in English/Spanish is preferred
Ability to work flexible hours is preferred
